Compliance Review Form (STD-21)
In accordance with the Commonwealth of Pennsylvania Contract Compliance Program Guidelines, M215.2, you are required to complete and submit this form. The information requested on this form will be used to determine a company's compliance with the Nondiscrimination Clause contained in the contract. This form can be obtained by going to http://www.dgs.state.pa.us and searching for the form in their Forms section.
Compliance Review Follow-up Form
Please submit a copy to DCNR's Contract Compliance Officer within one year of the date requested. This form can be obtained by clicking here
Prevailing Wage Requirements(LLC-25):
